A healthy relationship requires regular maintenance and self-awareness. Just as you check the dashboard of your car to ensure all systems are running smoothly, taking time to assess your relationship’s health helps you identify small issues before they escalate into major crises.

This self-assessment and quiz pack is designed to help you and your partner gain clear, objective insights into your relationship dynamics. By answering these questions honestly and sharing your results with each other, you can open up meaningful, non-defensive conversations about where your partnership is thriving and where it needs a little extra care.

Part 1: Pre-Relationship Self-Assessment (For Individual Reflection)

Instructions: Before assessing your partnership, take a moment to reflect on your own personal relationship habits, beliefs, and emotional patterns. Rate each statement on a scale of 1 (Never) to 5 (Always).

Score 80-100: Thriving Relationship.Your partnership is built on a highly resilient, emotionally secure, and loving foundation. You communicate effectively, maintain deep trust, and share a clear vision. Keep practicing these habits to protect your bond.
Score 50-79: Balanced with Growth Areas.You have a solid foundation, but there are specific areas where tension, disconnection, or communication breakdowns are creeping in. Look at your individual category scores to identify which areas need immediate focus.
Score 20-49: Significant Strain.Your relationship is likely experiencing a high degree of stress, emotional distance, or unresolved conflict. This is a clear signal that you need to slow down, rebuild emotional safety, and actively practice the connection tools in this workbook.

Part 3: Identifying Areas of Improvement Worksheet

Instructions: Sit down with your partner and compare your quiz results. Answer the following questions together in a calm, non-defensive environment:
